Patients Care Pathway

Rehabilitation can be defined as:
Leading patients from a state of medical dependence to a condition of functional dependence through an active medical, therapeutic and nursing process.

With this in mind, the team at RMC has developed a unique care pathway under a single care provider as illustrated by the following diagram:

Pre Admission Assessment

The RMC carries out personal and detailed pre-admission assessments to help each patient access the right placement and resources for a successful outcome of their planned admission with us.

REASSESSMENT
The Team at RMC has a policy of re-assessing each patient every 6 weeks. These assessments are attended by our dedicated interdisciplinary team fully inclusive of carers, in order to ascertain that the detailed goal planning process is fully implemented. We use the FIMFAM and disability rating scale as one method of measuring the outcome criteria.
